Organ and tissue donation is a generous gift that saves and enhances lives.

South Dakotans can register to be an organ and tissue donor by checking the box on their driver's license application or renewal form, and sharing their wishes with their family.

To add donor designation to your driver’s license between renewals, download the driver’s license application by clicking here and return the completed form to the address below. There is no charge to add ‘donor’ to your driver’s license between renewals. You will not receive a new driver’s license, but your donor designation will be added to the State database.
